REQUEST
This request is fOr approval of Amendment No. Two to C""tract No, OXI18151 with
Foundatioa Health arK! Psychcare Services, I~c. (fOrm",.ly OccupationaJ Health Services, Inc.)
fOr an additional one year term fOr continued Employee Assistance Plan (EAP) sen,ices.
RECOMMENDATIONS

UEClITlVE SUMMARY
The City bas provided employee .ssistance ser .... ices since 1983. This program is designed 10
pI'O\ide services included in labor agreements and help promote tl,e well-being and welfare of
City of Palo M" employees, arK! theTeby ~ on-the·job distraction and absenteeism. This
report recommends exleooUlg the tenD of the current agreement fOr an additional year at current
rates with no changes in the terms and cooc!ltions of the curren! contract. The lewl of senice
is excellent with no complaints being "".ived from employees.
