through thp-Ht!nan Services ResQurc.-e Allocation P:r-ocess (HSRAP). A
proposed reducticn of $41.682 in the tctal Human Ser.Jices cc,ntract
budget ...... as subrr.i tted to the Finance Comnittee. The Finance
Commi ttee then proposed an additional J percent increase in funding
over the 1991-1992 current year's budget. This motion was passed
on June. 9. 1392 and forwarded to Cou:J.cil as part of the 19'12-1993
budget. The attached amendments and agreements include contract
budgets ~ith the additJcnal 3 percent of funding. Funding
recom.'I'<Iendations utilizing Com1"'tunity D€'Jeloprnent Block Grant (CDBGj

THIS AMENDMENT to Contract No. C0006277 is ma.de and

the CITY OF PALO ALTO, a municipal corporation of Califor:1ia,
hereinafter referred to as ·CITY,~ and the PENINSULA AREA
INFORMATION AND REFERRAL SERVICES j INC., a nonprofit corporation
(Taxpayer Identification Number: 5.1-0192551) I 'Whose address is
3990 Ventura court, Palo Altc, CA, 94306, hereinafter referreo to
as "CORPORATION";

WHEREAS, both parties desire to rene,. said agreement for
an additional one-year term and to modify the sections set forth

NOW, THEREFORE, in consideration of their mutual
cov~nants, the parties hereto agree as tollo~s:
SECTION:t.~ The follol,iWing paragraphs of Contract No.
C0006277 are herel:ly amended to read as follo'.,s, as of July 1, 1992:
a. Paragraph 4(a).

CORpORATION for the full perforIIlance of its services under this
agreement in a total sum not to exceed Forty-four ThouEand Dollars
($44.000). This sum includes ~hirty-four Thousand Dollars
($34,000) in General Fund monies and Ten Thousand DQllars ($10,000)
in Community Development Block Grant ("CDBG=-) monies. Funds
committed under the CDBG program are contingent upon CITY receiving
the intended appropriat.ion "from the federal Department of Housing

"CORPORATION agrees that each agreement for service.s
from inde~ndent providers shall contain a provision substantially
as folloW's:
Provider shall provide CORPORATIon with a
certificate stating that he (or she) is currently in
compliance with all federal and State of california la~s
covering nondiscrimination in employment; that he (or
she) will pursue an affirmative course of action as
required by the Affirmative Action Guid.:!lines of the cit)'
of Pale Alto; and that he (or she) will not discriminate
in the emploYl'1ent of any person under this contract

MANAGER may request. The final semi-ann~al report shall cover not
only th~ preceding ~ix (6) months, bu~ also contract services for
the entire year. Each report shall be pr~pa~eQ in the form agreed
upon by the PRO.JECT MANAGER and CORPORATION. On reasonable notice,
CORPORATION shall grant the PROJECT ~~ACER access to ~ll
CORPORATION records, data, stat€~entsJ ~nd reports, but excluding
any confidential records or ~lient records which tend to identify
specific individuals.~

WHEREAS, both parties desire to amend said aqreement fer
an additional one-year term and to modify the sections set forth

NOW, THEREFORE) in consideration of their mutual

SECTION 1. The following paragraphs of Contract No.
C0001712 are hereby amended to read as follows, as of July 1, 1992:
a. Paragraph 4(b).
"(a) Specified~. This agreement shall commence
on July 1, 1992 and terminate on June 30, 1993, unless socne.r
terminated as provided herein.-

"(b) Compensation. CITY agrees ~o compensate
CORPORATION for the f\111 performance of its s~rvices under this

CITY shall pay CORPORATION 'Within fifteen (15) working days of

"(f) Audits. CORPORATION shall provide for
independent audit of its fiscal year transactions, records, anq
finan~ial reports at least every two (2) years~ The next audit
shall be completed by AUgust 31, 1993 and shall be conducted by a
certified public accountant according to generally accepted
acc!:Iunting procedures. The .-:;ertifled public accountant shall
submit the report to both parties. The cost of this audit shall be
borne by CORPORATION. CORPORATION shall also allo~ CITY to, in
CITY's sole discretion, cond"..lct any financial or other fonn of
audits of CORPORATION."

"18. NONDISCRIMJnR.~ No discrimination shall be
made in the employment of persons under this agreement because of

prior notificaton and documentation (excused absence forms) shall be dropped

shall be detenmined by the PACCC Executive Director and Contract Coordinator.
Excuse forms may include an excuse slip signed by the parent, glJardian or
appropriate doctor.

summarize the ch~dren's attendance at all PACCC facilities ar.d affiliates. PACCC
sdministrative personnel shall periodically review the forms and record keeping of

administration policies and proceduleS shail periodicaJ1y be updated. and enforced,
consisIant .... iIh the requirements of this contract. Exceptions to oompIiance wfth

RECITALS:
WHEREAS, Contract No. C0006273 'Was previously entere<3
into between CITY and CORPORATIon tor the provision of certain
counselinq services for younqer adolescents in the comm~~ity; and
WHEREAS, both p~rties desire to ren~w said agreement for
an additional one-year term and to Illodify the s:ections set forth
below;
NOW, THEREFORE, in consideration of their mutual
covenants, the parties hereto agree as follows:
SECTION 1. The following paragraphs ot Contr-act No.
C0006279 are hereby amended to read as follows, as of July 1, 1992:
a. Paragraph 4(a).

OT. July 1, 1992 and teI'lllinate on June 30, 1993, unless sooner
terminated as provided herein.w
b. Paragraph 5(b).
-Co) Compensation. CITY agrees to compensate
CORPORATION for the full performance of its services under this
agreement in a total sum not to exceed Forty-six Thcusand T"1I70
Hundred Fifty Dollars ($46 1 250). This sum includes Thirty-six
Thousand Two Hundred Fifty Dollars ($36,250) in General Fund monies
and Ten 'l'housand Dollars ($10,000) in COl!ll!'lunity Development Block
Grant '"CDBG~) monies. Funds committed under the CDBG program are
continqent upon CITY receiving the i~tended appropriation from the
federal Department of Housing anj Urban Development.-
c. 'l'he first subparagraph of Paragraph 18.
"18. NONDISCRIMINATION. No discrimination shall
be made in the employment of persons under this agreement because

d. The second eubparagraph of Paragraph 18 •

from independent providers shall contain a provision substantially

Provider shall provide CORPORATION ~ith a
certificate stating that he (or she) is currently in
compliance with all federal and state cf California laws

she) will pursue an affirmative course of action as
requiretj DY the Affirmative Action GUidelines Qf the City
of Palo Alto; and that he (or she) will not discriminate
in the employment of any person under this contract
because of race, color, national origin~ ancestry~ sex,
religion or disability of such person.-
SECTION 2. Subparagraph 19 (e) is hereby added to
Paragraph 19 of Contract No. CO006219 to read as follows, as of
July l~ ~992:

entitled "Federal Assu!'a~ces,· which is attached hereto and

SERVICES: Cor.tractor shall match funds granted by the City of Palo Alto under
this agr,ement to pro'wide two (2) fL!11-tim€ trained comunity counselors (one at
Gunn High Sdool and one at P .. 'iO Aito ,Yigh School) who shall address predictable
issues in adolescence on a regular, full-time basis at those locations and
increase student access to personal co:.!nsel ins services.
GOAL: To ensure the availability of a counse1o~ at each secondary school sit~
w~ose exclusive responsibility is to address the affective and socia-emotional
needs of secondary students anG their families including, but not 1,mtted tOI the
