Such desire ,-:;hall be indi.cat.ed by the tenants' si9natm:e on a form which contains the .statemen.t that the project may be converted to community housing if two-thirds of all adult t~:nants lawfully in possession so desire. The form sllall state that such tenant, by signing said statement, irrevocably const:n ts to such conversion. Such form s hal 1 al so state that conversion may require the tenants to find other housing and to relocate outside the City. 'I'he tenants' name and address sha1. l be printed on such form. No othe·r words or provisions shall appear on such form. Any tenant occupying a unit after the filing of such an application shall be informed in writing of the pen­ dency and consequences of the application prior to occupancy or execution of any rental or lease ngreement. Notwi th standing the above paragraphs of subsection ( c:), there shall be a six month moratorium commencing on the effective date of this ordinance (September 4, 1980) prohibiting the submittal of condominium con­ version applicationsr providedt however, that applications may be filed during said rnoratorium if a vacancy si.trplus exists within the meaning of this chapter.
